Once you have completed the online registration, it will prompt you to make an appointment with the district registration secretary, whose office is at Cloverleaf High School. In lieu of your appointment, please feel free to either scan or take a photo of the documents listed below and email them to the District Registration Secretary at [email protected]:
Birth certificate
Immunization records
A proof of residency (dated within the last 60 days). Acceptable proofs of residency: Utility bill, real estate tax bill, bank statement, recent receipt with parent's current address on receipt, pay stub, lease agreement or notarized residency affidavit
Parent/guardian ID
Divorce/custody papers (if applicable)
If you are living with someone, they will need to fill out a Residence Affidavit Form (see links below) and have it notarized. Please attach two forms of proof of residency in that individual’s name to the notarized Residence Affidavit and bring them to your appointment or send via email to [email protected].

The registration process cannot be completed until ALL required documents are provided.
Below is a list of additional documentation that will help with the scheduling process:
Most recent IEP, ETR or 504 Plan (if applicable)
Copy of last report card (grades K-8) or unofficial transcript (grades 9-12)
Standardized test scores
The Central Registration Office is located at Cloverleaf High School, 8525 Friendsville Road. Please come to the main front door of the building. You will sign in at the high school office and be directed to the registration office. Please click HERE for a map to Cloverleaf High School. If school is canceled due to inclement weather or for some other circumstance, the Central Registration Office will be closed. School cancelations are posted at www.CloverleafLocal.org.
